Requesting Health Records

You may request a copy of your health record by completing a Request of Information form. Please ask a member of our staff for a copy. A copy can also be found online.

To request medical health records from Family Care, Psychiatry and Behavioral Health Care, Women's Care Plus or the Mobile Clinic:

  • Submit a request in writing via U.S. Mail, fax or email.
  • Include contact information and a clear description of the records you are requesting.
