Non-Texas A&M Providers - Credentialing Process Steps

Step 1

Please provide the following documents:

  •  Curriculum Vitae
    • Must include currently employed position
    • Education, training and work history must include month and year [format MM/YYYY]
    • Must provide explanation for any gaps greater than three (3) months
  • Current license(s), DEA, Practitioner Controlled Substance Registration
  • Board certifications (if applicable)
  • Copy of masters/medical degree or college transcripts

Step 2

Please complete the credentialing application.

Step 3

Please complete the transfer of Authorization/Signatures document.
