Non-Texas A&M Providers - Credentialing Process Steps
Step 1
Please provide the following documents:
- Curriculum Vitae

- Must include currently employed position
- Education, training and work history must include month and year [format MM/YYYY]
- Must provide explanation for any gaps greater than three (3) months
- Current license(s), DEA, Practitioner Controlled Substance Registration
- Board certifications (if applicable)
- Copy of masters/medical degree or college transcripts
Step 2
Please complete the credentialing application.
Step 3
Please complete the transfer of Authorization/Signatures document.
