Texas A&M Providers - Credentialing Process Steps

Step 1

Please complete one of the following Texas A&M Health Credentialling Application packets:

Provide a copy of your licensure and a completed W9 statement. Required licensure and other documents can be found in the packet.

Email the aforementioned items to credentialing@tamu.edu.

Step 2

Please visit the Centers for Medicare and Medicaid Services website, and grant surrogate access.

Step 3

Please visit the Council for Affordable Quality Healthcare (CAQH) website and complete a provider registration. If you already have an existing CAQH profile, please email this information to our credentialing services staff.

Please forward any received notifications or communications received from payers or other related agencies to credentialing@tamu.edu
